NHS Membership North East and North Cumbria

To join the Library you will need to fill in our online application form.  You will need to provide one of the following as verification that you are currently employed by the NHS:

• A recent payslip (from the last 3 months) OR
• Current contract or Terms of Reference OR
• Letter from your employer on headed paper

Please note - we do not accept ID cards as verification because they are undated.

GPs and Dentists who work solely with NHS patients can show official headed practice notepaper giving name of applicant and stating that the practice treats NHS patients.

Please provide a passport-sized photograph (head and shoulders), which we will use to create your smartcard. You can attach this to your completed application form.

Please upload the documents prior to submitting the form.

NHS (Borrowing) customers are entitled to borrow 8 long loan items.  NHS Access customers are entitled to building access only.
